Blood is broadly divided into three types of cells; red blood cells which carry oxygen, white blood cells which fight infection and, platelets which help blood to clot. Platelets initiate the coagulation cascade which is an exquisite cascade of cellular events that leads to the formation of a blood clot. Platelet-rich Plasma (PRP) and Platelet-rich Fibrin Matrix (PRFM) are biological products defined as plasma with a platelet concentration above baseline.
The Process of Plasma Rich Fibrin Matrix (PRFM)
PRFM describes a method of producing a dermal filler from a patients own platelet and plasma. It produces a fibrin mesh to provide continued growth factor release. During a short office visit (30 minutes), a small amount of a patient’s blood is drawn into a vacuum collection tube containing a cell separator gel. The tube is then spun for six minutes to separate the blood into a suspension. The suspension is then transferred to a second vacuum tube containing a small amount of calcium chloride solution and fibrin polymerization begins. The resulting fluid can be injected into the face as a dermal filler or used topically with microneedles to stimulate skin tightening and production of new collagen.

PRFM contains highly concentrated platelets as well as the full complement of blood clotting factors, growth factors and cytokines (small proteins involved in cell signaling.) Studies have shown platelets and growth factors (GFs) derived from the patient’s own blood stimulate new blood vessel growth as well as enhance stem cell migration and proliferation. These effects stimulate fibroblast proliferation and, in turn, new collagen synthesis as well as the formation of adipose or fat tissue.
